2008年1月至2009年12月我院共收治1 305例确诊结核患者, 经过实施强有力的医院感染管理措施, 无一例护理人员发生医院内感染, 现将相关感染控制策略及防护基本要求总结如下。
1 资料与方法
1.1 一般资料
在我院结核科工作的护士共51名。2008年1月至2009年12月我院收治住院结核患者1 205例;男801例, 女404例;平均住院时间18 d;所有患者均符合中华人民共和国卫生部传染病标准专业委员会颁布的《传染性肺结核诊断标准》。
1.2 管理措施
1.2.1 增强自我保护意识, 预防感染
肺结核科收治的肺结核病人属呼吸道传染性疾病, 首先我们必须组织对本科室工作的医务人员进行相关知识点的学习和巩固 (包括实习的、进修的、轮转的) , 通过讲座、板报、业务学习等形式, 把临床护理工作中的各种职业性感染和损伤, 各种污染或感染的方式与途径, 及医院感染的危害性等等进行认真的讲课和指导, 让她们掌握正确的自我防护措施, 避免或减少各种危险因素的伤害, 同时让她们意识到严格的自我防护不仅使医护人员免遭疾病的侵袭和危险因素的伤害, 而且可减少医院内的交叉感染, 降低医院感染的发生率, 也是护士职业道德的要求。
1.2.2 掌握正确的自我防护技术
1.2.2.1 正确使用个人防护用具
(1) 口罩是防护呼吸系统病菌传染的重要用品。护理工作中严格按照操作规程要求戴好口罩。走入我们的工作区, 不管是否接触痰菌阳性, 还是痰菌阴性的病人, 都存在着潜在感染结核菌的危险, 我们都应戴好口罩。口罩使用要规范, 必须遮住口鼻, 一天更换2次。潮湿污染要及时更换, 处理完开放型肺结核咯血病人后要更换口罩。护理痰菌阳性的气管切开病人时要戴双层口罩。 (2) 正确穿着工作服, 要求衣着整洁, 扣好工作服衣领和袖口。每周换洗2次, 如被血液、痰液、胸水污染时应立即更换, 并要先将工作服消毒浸泡后再清洗。晨、晚间护理时, 为了避免整理床铺和进行生活护理时, 将床被表面的微生物、带菌尘埃污染空气或沉淀于护士工作服和口罩表面, 因此要求整理床铺时采用湿式扫床法, 同时进行开窗通风, 保持空气流通。每天下班时, 要求洗澡后回家。 (3) 进行下列操作时必须注意戴手套:护士在收集肺结核病人的各类标本时, 在接触病人血液、分泌物及排泄物时, 处理污染的敷料、器械及引流袋, 接触刺激性化学消毒剂, 配制细胞毒性化学药物等都必须戴手套, 一旦手套破损要及时更换。
1.2.2.2 严格洗手
医务人员的手是医院感染的重要传播媒介, 正确洗手可避免医院内交叉感染, 是预防自身感染最有效的措施。掌握使用各种洗手设施及在何种情况下需消毒剂泡手, 如紧急救治痰窒息、咯血窒息病人进行体位引流或用手清理呼吸道后, 或未戴手套接触了痰液、血液、胸水后, 或护理了爱滋病合并肺结核的病人后, 都应严格执行手的消毒, 培养良好的洗手习惯。
1.2.3 防止理化生物因素的伤害
结核菌的空气消毒主要是应用紫外线或电子灭菌灯每天照射1 h, 照射前要关闭门窗, 事先应做好病房的清洁工作。照射时工作人员应尽可能让能起床的病人避开, 因为紫外线照射时由于辐射的光化学效应, 臭氧吸收过多会导致恶心、呕吐、电光性眼炎, 皮肤可发生水肿、红斑、皮炎, 严重者可发生肺水肿、中毒等症状[2]。照射紫外线或电子灭菌灯结束后应立即打开门窗通风换气。甲醛的盐酸毒性、刺激性均较大, 在拖地、配制消毒液、熏蒸时动作应迅速, 减少开关熏箱的次数, 接触强酸、强碱、强消毒剂、挥发性气体应用防护手套及口罩, 操作过程中小心谨慎, 消毒后的物品应作相应处理。
1.2.4 锻炼操作技能, 严格遵守操作规程
操作熟练、动作协调可使误伤和污染的机会明显减少, 如给肺结核痰堵、咯血的病人吸痰, 特别是给痰菌阳性的气管切开病人吸痰、吸血、给危重病人做口腔护理、给呼衰的病人上呼吸机时, 护士一方面应戴好口罩, 另一方面应保持适当的距离, 尽量避免直对污染面, 强调做到操作熟练迅速, 有效地缩短被污染的机会。
1.2.5 心理因素的防护

适当减轻护士的工作强度, 降低心理压力, 管理者多关注医务人员的心理健康, 通过改善、优化工作环境, 合理地调配人力资源, 改进排班制度, 保证足够的睡眠和休息, 提高护士的待遇, 缓解精神压力, 满足心理需求。
1.2.5.2 生理因素的防护
加强体育锻炼, 提高身体抵抗力, 降低染病的几率。通过培训、监督和检查, 使护理人员掌握正确的自我防护技术, 意识到临床工作中做好自我保护和消毒隔离工作的重要意义, 并注意生活起居、劳逸结合以利更好地工作。在减少病人发生医院感染的同时, 保持自身健康, 把医院感染率和职业危害性降到最低限度, 才能避免像非典中医务人员染病的悲剧不再重演, 也才能使医务人员更好地服务于社会[3]。
1.3 结果
1 305例结核确诊患者平均留院时间为18 d, 经精心的治疗与护理, 实施一系列上述强有力的医院感染管理措施, 所有医护人员均未发生院内交叉感染, 患者均好转出院。
2 讨 论
结核病是由结核分支杆菌 (mycobacterium tuberculosis) 引起的慢性传染性疾病, 可累及全身各个器官, 其中尤以肺结核最为多见。痰中排菌的肺结核病人属传染性肺结核, 是造成社会结核病传播和流行的传染源, 为首要控制对象。传染性肺结核病人在咳嗽、打喷嚏、大声说话等过程中产生含结核分支杆菌的微滴核, 它可随气流飘浮在室内引起结核病传播。要劝阻病人不可对着人咳嗽或打喷嚏, 咳嗽时要用手帕、口罩等掩盖口鼻, 不随地吐痰。痰液可用焚烧、煮沸、化学药品灭菌处理。另外, 加强室内空气流通, 日光和紫外线照射, 采用抗结核药物治疗病人等都可有效地减少与防止结核病的传播。我院通过在接诊和治疗过程中, 采取了严格的感染管理防护措施, 如正确洗手, 以往对该项措施重视不够, 造成医源性传播及自身感染无法绝对避免。通过各级高年资护师巡视督促, 在各种洗手设施旁标示正确洗手方法, 使该项措施真正落实, 杜绝了该病在医院医源性传播与流行的途径。为减少结核等传染病的传播和再次感染, 疫情蔓延, 降低发病率, 需全社会共同参与并落实, 建立有效的切断传播途径为主的综合性预防控制措施, 控制结核疫情的蔓延, 大幅度降低结核发病率, 保护人民群众的身心健康。
摘要:医务工作人员在工作区域也随时有被病原体感染的危险, 国际上早在20世纪80年代初就开始了关于医务人员职业暴露的相关研究, 护士是最危险的人群, 肺结核科护士的自我防护问题越来越受到关注, 做好自我防护不仅可以避免疾病的侵袭, 同时也能减少院内交叉感染的发生。作者针对结核科护士在临床工作中的自我防护问题进行探讨, 以增强护士的职业防护意识, 健全职业安全防护措施, 降低职业危害程度。

工作倦怠是指服务于助人行业的从业者因为不能有效地应对工作上持续不断的压力而产生的一种包括情绪衰竭 (emotional exhaustion, EE) 、去人性化 (depersonalization, DP) 和个人成就感 (reduced personal accomplishment, PA) 在内的综合症状[1]。现已成为职业心理学的研究热点问题之一[2]。
由于医疗服务行业本身的高风险性和特殊性, 工作倦怠现象在护理人员身上表现得尤为明显, 护士在工作倦怠十大高发职业中名列第一, 并且有研究发现[7], 不同科室护士的工作倦怠是不同的。产房是医院对孕产妇实施助产分娩、手术治疗、检查、诊断并担负母子二人生命安全抢救工作的重要场所, 产房护理人员肩负着母婴健康和安全的重任[3]。在产房, 正常分娩的产程观察、接生、新生儿的处理基本上都是由产房护士来独立完成, 医生只在有异常情况的时候才作出干预措施, 与病房护士相比, 产房护士的工作自主性、独立性更强, 同时也要求有更高的心理、业务能力来独立完成工作, 如果没有强大的自我效能感作支持, 可能容易导致严重的倦怠。本调查旨在探讨产房护士的工作倦怠情况;自我效能感现状以及工作倦怠、自我效能感两者相互间的关系, 为日后降低产房护士工作倦怠感所需采取的个体及集体干预措施提供依据, 以进一步提高产科护理质量。
1 对象与方法
1.1 研究对象
以南京市5家三甲医院的全体在职产房护士为研究对象。共发放问卷102份, 回收有效问卷98份, 问卷回收率为96.1%。研究对象均为女性, 年龄21岁~52岁。
1.2 研究工具
1.2.1 自编产房护士一般情况调查表
包括性别、年龄、职务、职称、最高学历 (学位) 、所学专业、婚姻状况、生育情况、劳动人事情况、子女状况等指标。
1.2.2 Maslach编制的工作倦怠量表 (MBI) [1]
量表由22个条目组成, 包括3个维度, 情绪衰竭 (9个项目) 、去人性化 (5个项目) 和个人成就感维度 (8个项目) 。所有项目在回答时均采用从0到6的七点计分法:0分~代表“从来没有出现”, 6分~代表“每天都出现”, 3个分量表的得分由各自所含条目的得分相加, 但3个分量表的得分则相互独立, 不能相加。其中情绪衰竭和去人性化的分量表采用正向计分法, 分量表的平均数越大, 说明工作倦怠程度越强;个人成就感分量表采用反向计分法, 平均数越小, 说明其工作倦怠程度越高。本研究使用的是李小妹翻译的中文版, 经检测, 其信度系数cronbach’s α为:总量表0.93、情绪衰竭0.91、去人性化0.81、个人成就感0.84[4]。
1.2.3 自我效能感量表
一般自我效能感量表 (general self-efficacy scale, GSES) 共10个题目, 涉及个体遇到不同困难和麻烦时的自信心。该量表采用1级~4级计分法。完全不正确计1分, 完全正确计4分, 均为正向计分。得分越高表示一般自我效能感水平越高。
1.3 统计学方法
本研究所有数据经整理、编号、输入计算机后, 采用SPSS17.0软件包进行统计分析。
2 结果
2.1 产房护士的工作倦怠情况 (见表1)
2.2 产房护士的自我效能感
产房护理人员自我效能感得分为25.30分±5.40分;最高分38分, 最低分14分。
2.3 产房护士工作倦怠感与自我效能感的关系
将产房护士自我效能感得分与工作倦怠感得分进行Pearson相关分析。 结果显示:自我效能感得分与情绪衰竭感维度得分呈负相关 (P<0.01) , 自我效能感得分与个人成就感的维度分呈正相关 (P<0.05) , 自我效能感得分与去人性化得分呈负相关 (P<0.05) 。
3 讨论
3.1 产房护士的工作倦怠感较高
Maslach在其研究中将其样本正态分布的上1/3考虑为高度、中1/3考虑为中等、下1/3为低度来区分其倦怠程度。情绪衰竭维度得分19分以下者为低度倦怠, 19分~26分者为中度倦怠, 26分以上者为高度倦怠;去人性化维度得分6分以下者为低度倦怠, 6分~9分者为中度倦怠, 9分以上者为高度倦怠;个人成就感维度得分39分以上者为低度倦怠, 34分~39分者为中度倦怠, 34分以下者为高度倦怠。现将本次调查结果产房护士工作倦怠各维度平均分与其对照可以发现产房护理人员的情绪衰竭处于中等水平, 去人性化处于低度水平, 个人成就感处于高度水平。可见产房护士在日常工作中的倦怠感较高, 并且主要表现在个人成就感方面。由此可见:要缓解产房护士的工作倦怠程度, 最关键的是应提高其个人成就感。
3.2 产房护士的自我效能感
本调查发现产房护士的自我效能感最高分为38分, 最低分14分, 得分为25.26分±5.06分, 属于中等水平。尹心红等[5]于2009年通过对湖南7家三级甲等医院407名临床护士的调查发现其自我效能感得分为26.67分±5.30分。本调查低于尹心红等调查的护士组得分。其原因可能是由于产房工作的高技术性、高风险性, 使产房护士较普通临床护士自信度低。
3.3 产房护士的自我效能感与工作倦怠感密切相关
相关性分析结果显示, 产房护士自我效能感与情绪衰竭感、去人性化均呈负相关, 自我效能感与个人成就感呈正相关。说明自我效能感越高的个体, 工作倦怠感越低。自我效能感这一概念是美国著名心理学家班杜拉于1977年首次提出的。所谓自我效能感是指“人们对自身能否利用其所拥有的技能去完成某项工作行为的自信程度”[6]。大量的研究结果显示:①具有较低自我效能感的人, 在生活中面临挑战时, 往往将其视为一种威胁, 因而采取回避的态度;具有较高自我效能感的人, 对于环境中的挑战则采取积极的应对态度, 在他们看来, 正是这些挑战为其提供了各种学习新技能的好机会。②自我效能感会影响人们动机性努力的程度:当人们感觉自己在某项工作上有较高自我效能时, 就会干得更加卖劲, 而如果认为自己在某项工作上的效能较低时, 就不会付出那么多的努力[7]。③自我效能感还能影响人们的情感过程, 在面临可能的危险、不幸、灾难等恶性情境条件时, 自我效能感决定了个体的应激状态、焦虑反应和抑郁的程度等情感过程, 这些情绪反应又会反过来通过改变思维过程而影响个体的活动及其功能发挥。
4 对策
4.1 尝试产房护士特殊岗位准入制 (包括按个性定岗、按自我效能感将合适的人放到合适的岗位) 产房由于其工作的特殊性, 并非所有的护士都适合去产房工作, 所以医院管理者在决定护士岗位的时候可参考每位护士的自我效能感、兴趣、对岗位的热爱程度。 护士在进行岗位选择时, 除了参考收入、工作环境等硬件条件, 也应该考虑自己的各方面能力、优势和缺点, 选择能充分发挥自己长处的岗位。
4.2 努力提高在职产房护士的自我效能感, 降低其工作倦怠感 可用以下方式提高产房护士自我效能感:利用以往成功经验鼓励、激励护士;在产房护士间树立自信、乐观、敬业的榜样来影响自我效能感低的护士, 使其逐渐增强工作的自信度;社会劝说方式[8]。
4.3 在高等学校设置助产士专业 通过在高等学校设置助产士专业提高临床助产士的整体素质, 增强其自我效能感, 从而减轻其工作倦怠感。

1对象与方法
1.1研究对象
采用随机整群抽样方法于2015年4月抽取浙江省15所三级医院的手术室在职护士为研究对象。纳入标准:从事手术室护理一线工作且一线工作时间至少1年;在职注册护士;自愿参与本次研究。排除标准:未注册护士; 非手术室一线工作护士;手术室工作时间未满1年;实习生、进修生、轮转护士。本次研究共发放问卷2100份, 剔除不合格问卷及答题倾向过于集中的问卷102份,有效问卷为1988份,问卷有效回收率为94.67%。其中:男性428名,女性1460名;年龄23-43岁, 平均30.56±9.63岁,23-29岁1060名,30-39岁860名,40岁以上者68名;护龄1-25(10.66±10.68)年, 1-5年658名,5-10年713名,11-20年535名,20年以上者82名;学历:大专573名,本科1373名,研究生42名;职称:初级1248名,中级648名,高级92名;婚姻状况:未婚759名,已婚1165名,离异或丧偶64名;人事关系:在编447名;非在编1541名。
1.2研究工具
1.2.1基本社会人口学调查表。由研究小组在文献查阅基础上自行设计编制,包括性别、年龄、护龄、学历、 职称、婚姻状况和人事关系等。
1.2.2护士工作沉浸问卷。采用张莉莉[4]修订的急诊科护士工作沉浸体验问卷,该问卷各维度Cronbah’s α系数为0.707-0.784,具有较高的内容效度和信度。包括5个维度:体验能够激发自身的工作动力、工作中有清晰的目标并伴时间感的改变、行动与意识融合、了解各种进程及自己能够克制工作任务、专注于当前任务,一共27个条目,采用Likert6级计分,得分范围为27-162,分数越高说明护士工作沉浸越高。
1.2.3核心自我评价量表。采用杜建政等[5]修订的核心自我评价量表,该量表Cronbah’s α系数为0.83,分半信度为0.84,包括自尊、控制源、一般自我效能和情绪稳定等4个维度共10个条目,采用Likert5级计分,分数越高说明护士核心自我评价越高。
1.3统计学方法
本研究所得数据使用SPSS 12.0处理与分析, 采用统计描述、t检验、 方差分析、 P e a r s o n相关性分析,以P<0.05差异有统计学意义。
2结果
2.1手术室护士核心自我评价、 工作沉浸得分情况
调查结果显示,手术室护士核心自我评价得分为36.52±6.31分,总体处于对自我核心评价中等水平。手术室工作沉浸得分为118.68±25.34分,得分指标值为73.04%,总体处于中等水平,各维度指标得分由高到低分别为:工作中有清晰的目标并伴时间感的改变(27.47±4.84),专注于当前任务(18.02±3.96),了解各种进程、自己能够克制工作任务(26.27±4.99),行动与意识融合(21.70±5.36),体验能够激发自身的工作动力(25.21±5.28),均处于中等水平。

3讨论
3.1手术室护士核心自我评价特点分析
本研究发现手术室护士核心自我评价得分为36.52±6.31分,处于中等水平,高于胡桂春等[6]调查的226名三乙综合医院临床一线护士的核心自我评价,可能是本研究被调查护士均在三级医院手术室工作这一群体性有关。手术室是一个紧张、快节奏、高技术、高风险、超负荷的封闭环境, 多学科融合形成的复杂体系,较病区而言有更多的困难、更具有挑战,在手术配合和抢救中扮演着重要角色。 为了给患者提供包括身体、情感、精神和心理等复杂需求,手术室护士需要更多的知识和技能为病人提供安全护理,他们具有较高的核心能力,如专业品质、围手术期护理能力、批判性思维能力、管理能力、交流沟通能力和专业发展能力等[7]。这些直接反映在其护理患者和手术配合时能够表现出更明确的目标或标准,熟悉各种新技术、新业务的开展以及新设备的引进,有针对性地评估他们工作从长处或短处,思考“为什么”并反馈后进行相应的行为和认知修订,形成相当积极的自我认知和评价[8],进而保持自尊、控制源、一般自我效能和情绪稳定,更积极地应对方式和较低的工作倦怠,从而提高护理的有效性[9],促使自我保持较高的自尊和自信,形成良性循环。
3.2手术室护士工作沉浸特点分析
本研究发现, 手术室护士工作沉浸为1 1 8 . 6 8 ± 2 5 . 3 4分, 与张莉莉等[4]研究147名急诊科护士得分(112.14±21.23)比较,差异有统计学意义。同时“工作中有清晰的目标并伴时间感的改变” 维度得分最高, “ 体验能够激发自身的工作动力”维度得分最低。相对于急诊科的护理环境而言,手术室护理工作围绕着术前、 术中和术后展开, 其专业性、技术性和知识性相对集中,护士与患者“一对一”的护理模式促使护士必须有清晰的目标和专注于当前任务,能够深入护理工作,而且需要随着患者各项指标的变化及时改变。手术室强调团队合作,随着护士专业能力的不断提高,其地位也逐日提升, 能够有能力提供有益的意见和观点。 然而在目前的医疗大环境下,大部分医院仍然“重医轻护”,个人价值得不到充分的关注和肯定,护士往往疲于倒班、抢救、应激和自我防御等, 获得的经济、理想、机会等承诺均不高[10],付出不能等同回报,一定程度上磨灭护士积极的工作态度和工作投入,因而往往无法在工作中激发自身工作动力和活力。护理管理者应不断地激发护士工作体验和动力,提高其工作技能以适应岗位需求, 让 “ 工作”与“行动”契合,以提高护理工作绩效[11]。
3.3手术室护士核心自我评价与工作沉浸相关性
手术室护士核心自我评价各维度与工作沉浸呈正相关,即护士的工作沉浸体验越高,核心自我评价水平越高。研究认为核心自我评价高的个体对自身能力和价值所持有的评价较高,对自我有着较为积极的自信[1],在面对困难时不退缩、不气馁,能较强地坚持,“我是有价值的”“我从不焦虑”“生活中的事情在我的掌握之中”“我成功完成了任务”,会以积极的信念主动影响周围环境。而且研究发现,个人在某些工作核心自我评价高时,这个人在实际从事这项工作时也会更加得心应手。手术室的特殊护理环境,护士不得不承受着担心工作中出现差错事故、经常倒班、工作量大、工资及其他福利待遇低、护理的病人病情过重、担心护理的病人会突然死亡等多源压力,护士必须面对护理应变和刺激、压力和满意之间的平衡。而对工作环境的健康沉浸体验是一种积极的心理状态,当一个人沉浸在工作中时,个体会体验到幸福并主动提高工作认识和工作动机效能, 个体必然能够运用技能预知挑战处理问题,逐渐地掌控工作中的能力和活动本身的挑战[12]。
一个组织活动的专注和积极态度对员工的工作沉浸和健康有着重要意义[13]。本研究发现手术室护士的工作沉浸整体水平仅处于中等,因而仍有加大提升空间, 护理管理者需完善其人格特征,从管理支持、团队行为、资源保障、质量管理等方面有效提高组织氛围,减少刻板印象,加强团队建设,激发护士潜能发挥、满足其成长发展、自我表现和价值实现的需求,获得多的积极暗示、兴趣、体验、动力,从而提高其坚持不懈努力的毅力[14],以积极的工作接触、工作表现和工作成绩降低其工作带来的情绪匮乏感、工作冷漠感、个人工作无成就感等负性情绪。同时,管理者应考虑到手术室护理工作的特点,认真分析可能影响其沉浸的相关因素及后果,以奖励支持、组织支持和监督支持等参与模式提高护士工作沉浸[15], 指导其正确对待问题、正确对待负性情绪、正确对待人际关系等,以持久的活力、奉献和沉浸更好地投入和专注注护护理理工工作作。。
